Dear colleague,
An outbreak of pertussis occurred in New York City from August 2011 through April 2012. Since then, case counts have decreased but remain elevated above the prior baseline. The New York City Department of Health and Mental Hygiene (NYC DOHMH) is conducting a brief survey to understand whether increased provider awareness and changes in provider practices are related to the increase in reported pertussis. We ask your assistance with completing the survey, which will take less than 5 minutes. Thank you in advance for your participation.
